Rates
$130 per 50-minute session. Accepted forms of payment include all major credit cards, medical spending debit cards, cash, or check.
Insurance
I am currently accepting clients with Anthem Blue Cross Blue Shield. For clients with other insurance providers, services may be covered by your health insurance or your employee benefit plan. Documentation for insurance benefits will be provided at the client's request.
Cancellation Policy
On occasion, a situation may arise which prevents you from keeping a scheduled appointment. As a courtesy, please notify me 24 hours in advance of your appointment if you cannot keep it. Except in emergency situations, you may be required to pay one-half the current hourly fee. You will be personally billed for not showing up to an appointment.
